Below is a chart listing the forms parents should have returned to Overbrook by July 18, 2011. Please note that some forms you will have to download, print and return to Overbrook; others you can fill out online. Also, please note that the supply list does not require you to fill out and return a form but does require that you send in a check for your child's school supplies.
Optional forms are also included for athletic participation, band, and private music lessons if you are interested in those programs. Finally, medical forms for the clinic are available for those who need these for children with allergies or special medication. These forms must be signed by a physician.

* New parents -- If you plan to volunteer as a room parent, coach, Scout leader, classroom/library helper, or field trip driver, please click on the two links in the "New Parents" column in the table above. The first is a required background check you will need to complete and the second is a Safe Environment Power Point Program, which is required viewing. Please print out the acknowledgement form after viewing the Power Point and return it to the office if you plan to volunteer.
